Physical Information Required By SCISA
Physical: Every student-athlete must have a completed physical before he/she is allowed to participate, practice, or play any school sport. A physical is valid for one calendar year from the date of the physical.
Pre-Participation Medical and History Form: A family history and questionnaire shall be completed prior to seeing the physician and presented to the physician.

Parents Permission Form: Every student-athlete must have a current and completed Parent’s Permission Form on file.

Warning of Risk: Every student-athlete must have a current & completed Warning of Inherent Risk Form and Concussion Awareness Form on file.

New Student / Transfer Form: Required of any student-athlete who is new to the school for the current school year or who transfers after the start of the current school year.
